The 41-year-old motorist, from Sans Souci, was caught as police conducted random breath tests on the Princes Highway as part of Operation Saturation.
Just after 5.30pm on Sunday, officers stopped a Honda Civic after the driver allegedly attempted to avoid the RBT by turning into a nearby street.
He returned a negative breath test but gave officers false details and further checks enabled them to identify him and discover he was disqualified from driving until 2053, NSW Police said.
The man was given a Field Court Attendance Notice for the offences of driving whilst disqualified and providing false details.
He will appear at Kogarah Local Court on 1 April 2014.
